Responsibilities
- Provide personalized in-home care including meal preparation, medication reminders, and mobility assistance
- Monitor client health status using telehealth platforms and digital health records
- Coordinate with family members and healthcare providers via remote communication tools
- Assist with daily living activities while maintaining client dignity and independence
- Document care notes and client progress through our mobile care management app
- Transport clients to appointments using personal vehicle (mileage reimbursed)
- Implement personalized care plans developed by our clinical team
Qualifications
- Minimum 1 year professional caregiving experience or equivalent training
- Valid Wisconsin driver's license with clean driving record
- CPR/First Aid certification or willingness to obtain
- Reliable smartphone with data plan for remote coordination
- Ability to pass background check and drug screening
- Compassionate demeanor with excellent communication skills
- Basic proficiency with digital health records and telehealth tools
- Flexibility to work days, evenings, weekends, and holidays
