Responsibilities
- Provide telehealth nursing services to patients across Oakland area facilities
- Conduct virtual patient assessments and develop personalized care plans
- Collaborate with local healthcare teams to ensure seamless patient transitions
- Monitor patient progress remotely and adjust interventions as needed
- Document all clinical encounters accurately in electronic health records
- Participate in virtual case conferences and quality improvement initiatives
- Maintain HIPAA compliance during all telehealth interactions
Qualifications
- Active RN license in California (or compact state)
- Minimum 2 years of acute care experience
- BLS and ACLS certifications required
- Experience with telehealth platforms and remote monitoring systems
- Strong assessment and critical thinking skills
- Excellent communication and documentation abilities
- Ability to work independently in a virtual environment
- Flexibility to travel to local facilities as needed
