Responsibilities
- Conduct comprehensive patient assessments via telephone and in-person to determine medical urgency and prioritize care needs
- Utilize clinical judgment to accurately categorize patients using standardized triage protocols (ESI, CTAS)
- Coordinate seamless communication between patients, families, and multidisciplinary care teams
- Document detailed patient encounters using Epic EHR systems with precision and timeliness
- Provide evidence-based health education and discharge instructions to patients and families
- Collaborate with physicians and specialists to ensure timely interventions for critical cases
- Maintain strict HIPAA compliance and confidentiality standards in all patient interactions
Qualifications
- Active California RN license with minimum 3 years clinical experience
- BLS certification required; ACLS and TNCC preferred
- Proven expertise in telephone triage protocols and emergency assessment
- Exceptional critical thinking and decision-making abilities under pressure
- Strong proficiency with electronic health record systems (Epic preferred)
- Excellent communication skills with diverse patient populations
- Ability to thrive in fast-paced, high-volume healthcare environment
