Responsibilities
- Perform comprehensive patient assessments via phone and in-person triage to determine acuity levels and appropriate care instructions.
- Utilize Electronic Medical Records (EMR) systems to document patient interactions, symptoms, and triage decisions accurately and timely.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Provide patient education and discharge instructions to ensure understanding of treatment plans and follow-up care.
- Evaluate and prioritize incoming patient calls, ensuring critical cases are addressed immediately.
- Monitor and report adverse events or trends in patient symptoms to the clinical leadership team.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Pennsylvania.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 2-3 years of experience in a triage, urgent care, or emergency department setting.
- Strong knowledge of pathophysiology, pharmacology, and clinical assessment skills.
- Excellent verbal and written communication skills with a focus on patient empathy.
- Proficiency in using EMR software and computer systems.
