Responsibilities
- Conduct Comprehensive Triage: Evaluate patient symptoms via phone or video consultation to determine the appropriate level of care and urgency.
- Medical Decision Making: Utilize provided diagnostic equipment to assess vital signs and patient conditions, providing accurate medical advice and treatment plans.
- Equipment Coordination: Manage the dispatch, delivery, and setup of medical equipment for home care patients, ensuring readiness for the nurse's visit.
- Patient Education: Educate patients and families on the proper use of provided equipment and preventative health measures.
- Documentation: Maintain precise, up-to-date electronic health records (EHR) regarding patient interactions and triage outcomes.
- Collaboration: Liaise effectively with physicians, specialists, and other healthcare professionals to ensure continuity of care.
Qualifications
- Licensure: Current, active Registered Nurse (RN) license in the state of Pennsylvania.
- Experience: Minimum of 2-3 years of experience in triage, telehealth, or emergency nursing.
- Technical Skills: Proficiency in using electronic health record systems (EHR) and medical diagnostic equipment.
- Communication: Exceptional verbal and written communication skills with a focus on clarity and empathy.
- Decision Making: Strong critical thinking and decision-making abilities under pressure.
