Responsibilities
- Assess patient calls via phone/telehealth to determine urgency and appropriate care pathways
- Collaborate with physicians to develop patient care plans and interventions
- Maintain accurate electronic health records (EHR) documentation
- Provide health education and guidance to patients and families
- Coordinate patient transfers and emergency department referrals
- Adhere to HIPAA compliance and hospital protocols
Qualifications
- Active NYS RN license with 2+ years clinical experience
- BLS and ACLS certifications required
- Strong assessment and critical thinking skills
- Excellent written/verbal communication abilities
- Experience with EHR systems (Epic preferred)
- Ability to work independently in fast-paced environments
