Responsibilities
- Conduct Comprehensive Assessments: Utilize advanced triage protocols to evaluate patient symptoms via phone and video, determining the urgency of care required.
- Direct Care Pathways: Effectively route patients to the appropriate level of care, including urgent care, primary care, or emergency services, ensuring safety and efficiency.
- Clinical Documentation: Accurately and timely document all triage interactions, patient history, and care recommendations within our Electronic Health Record (EHR) system.
- Provider Collaboration: Consult with physicians and advanced practice providers to discuss complex cases and receive clinical guidance.
- Health Education: Provide patients with clear, actionable health education and self-care instructions to manage non-emergency conditions.
- Policy Adherence: Strictly adhere to HIPAA regulations and hospital triage guidelines to maintain patient confidentiality and legal compliance.
Qualifications
- Licensure: Active and unencumbered Registered Nurse (RN) license in the State of California.
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 3-5 years of nursing experience, with specific experience in triage or telephone nursing preferred.
- Communication: Exceptional verbal communication skills with the ability to convey complex medical information clearly and empathetically.
- Technical Proficiency: High comfort level with computer systems and telehealth platforms (e.g., Epic, AthenaHealth).
- Decision Making: Strong critical thinking skills and the ability to make rapid, accurate clinical decisions under pressure.
