Responsibilities
- Conduct thorough patient assessments via phone and in-person to determine the appropriate level of care.
- Evaluate symptoms, provide medical triage, and recommend treatment plans or referrals.
- Facilitate patient access to physicians, specialists, and emergency services when necessary.
- Document all patient interactions and medical history accurately in the electronic health record (EHR).
- Collaborate with the clinical team to ensure continuity of care and follow-up.
- Educate patients on disease prevention, medication management, and self-care strategies.
- Maintain strict adherence to HIPAA regulations and clinical protocols.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Colorado.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 2-3 years of experience in triage, emergency room, or urgent care nursing.
- Strong clinical judgment and critical thinking skills.
- Excellent verbal and written communication skills.
- Ability to remain calm and professional during high-stress situations.
- Proficient in using electronic medical records and telehealth platforms.
