Responsibilities
- Conduct comprehensive telephone and digital triage assessments for patients presenting with acute and chronic symptoms.
- Apply clinical decision-making protocols to determine patient urgency and recommend appropriate courses of action.
- Provide detailed patient education, medication reconciliation, and self-care instructions.
- Collaborate seamlessly with physicians, nurse practitioners, and clinical staff to ensure continuity of care.
- Document all interactions, assessments, and care plans accurately and promptly in the Electronic Health Record (EHR).
- Maintain strict adherence to HIPAA regulations and company privacy policies at all times.
- Identify trends in patient inquiries and provide feedback to improve service quality.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the State of New York (or compact state eligibility).
- Minimum of 2 years of recent acute care or triage experience preferred.
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) required.
- Strong proficiency with Electronic Health Records (EHR) systems (e.g., Epic, Cerner, AthenaHealth).
- Excellent verbal and written communication skills with the ability to de-escalate patient concerns effectively.
- Demonstrated ability to work independently in a remote setting while maintaining high clinical judgment.
