Responsibilities
- Conduct comprehensive phone and telehealth assessments to evaluate patient symptoms and medical history.
- Apply clinical protocols and critical thinking to prioritize patient care needs and determine appropriate disposition.
- Facilitate seamless communication between patients, physicians, and specialists to ensure continuity of care.
- Document all triage encounters accurately and in real-time within the Electronic Health Record (EHR) system.
- Provide patient education on self-care, medication management, and preventive health measures.
- Collaborate with the nursing and medical staff to address urgent care concerns promptly.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the state of Arizona.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Minimum of 2 years of recent experience in a triage, urgent care, or telehealth setting.
- Strong interpersonal and communication skills, with the ability to de-escalate patient emotions effectively.
- Proficient in using EMR/EHR systems and computer applications for documentation.
- Ability to work independently while maintaining strict adherence to medical protocols and standards.
