Responsibilities
- Perform comprehensive initial assessments of patient symptoms via phone or in-person to determine medical acuity and urgency.
- Utilize evidence-based protocols to prioritize patient care and route appropriate cases to the Emergency Department or Urgent Care.
- Document patient interactions, assessments, and care plans accurately and promptly in electronic health records (EHR).
- Communicate effectively with patients, families, physicians, and other healthcare providers to ensure continuity of care.
- Educate patients on self-care instructions, medications, and follow-up appointments to prevent readmissions.
- Monitor queue performance metrics and assist in the continuous improvement of triage processes.
Qualifications
- Current, valid Registered Nurse (RN) license in the State of California.
- Certification in Advanced Cardiovascular Life Support (ACLS) and Basic Life Support (BLS) required.
- Minimum of 1-2 years of experience in emergency medicine, critical care, or triage nursing preferred.
- Strong clinical judgment, critical thinking, and decision-making skills under pressure.
- Excellent verbal and written communication skills with the ability to de-escalate patient anxiety.
- Proficiency with electronic health record systems (Epic, Cerner, or similar).
