Responsibilities
- Conduct thorough patient assessments via phone and triage software to determine acuity levels.
- Determine the urgency of patient symptoms and recommend appropriate care paths or emergency services.
- Document patient interactions accurately and in real-time using EMR systems.
- Collaborate with physicians, nurse practitioners, and other care providers to ensure seamless patient transitions.
- Provide clear, empathetic instructions to patients regarding home care and medication management.
- Identify potential emergencies and initiate dispatch protocols immediately to ensure patient safety.
- Participate in ongoing training to stay updated on clinical guidelines and triage protocols.
Qualifications
- Active and unencumbered RN license in the State of California.
- Graduation from an accredited nursing program (Associate’s Degree in Nursing or Bachelor of Science in Nursing preferred).
- Strong communication and interpersonal skills with a focus on empathy and clarity.
- Ability to remain calm and make quick, critical decisions under pressure.
- Proficiency in computer systems and Electronic Medical Records (EMR).
- Willingness to work flexible shifts, including evenings and weekends.
