We are seeking a dedicated and skilled Triage Nurse to join our elite care team at Unity Medical Center. Based in the vibrant city of Minneapolis, MN, you will play a pivotal role in our patient intake and care coordination process. In this high-volume role, you will serve as the first point of contact for patients, utilizing your clinical expertise to assess symptoms, determine acuity, and direct patients to the appropriate level of care. Responsibilities
- Conduct Comprehensive Patient Assessments: Utilize evidence-based guidelines to evaluate patient symptoms via phone or telehealth, determining the appropriate level of care and urgency.
- Coordinate Care Continuum: Facilitate seamless referrals to providers, specialists, or emergency services when necessary, ensuring timely follow-up.
- Documentation & Reporting: Maintain accurate, detailed medical records in the electronic health record (EHR) system, documenting all triage interactions and outcomes.
- Patient Education: Provide clear, empathetic health education and instructions regarding medication management and self-care strategies.
- Operational Support: Assist in the development and implementation of triage protocols to improve departmental efficiency and patient flow.
- Quality Assurance: Participate in quality improvement initiatives and regular case review to ensure compliance with medical standards and hospital policies.
Qualifications
- Licensure: Current and active Registered Nurse (RN) license in the State of Minnesota.
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Experience: Minimum of 2-3 years of clinical experience in a hospital, urgent care, or telephone triage setting.
- Certifications: CEN (Certified Emergency Nurse) or CTRN (Certified Triage Registered Nurse) certification is highly desired.
- Skills: Proficient in electronic health records (EHR) systems (e.g., Epic, Cerner) and strong computer literacy.
- Communication: Excellent verbal and written communication skills with the ability to de-escalate patient anxiety.
