Responsibilities
- Assess patient symptoms via phone and determine appropriate care urgency
- Collaborate with physicians to develop treatment plans
- Maintain accurate electronic health records (EHR) documentation
- Provide education on home care and preventive measures

- Coordinate with specialty departments for seamless patient flow

Qualifications
