Responsibilities
- Perform initial patient assessments via phone and secure messaging to determine the urgency of medical needs.
- Utilize clinical protocols to triage patients and recommend appropriate care pathways (ER, Urgent Care, Home Care).
- Collaborate with physicians and care teams to facilitate seamless patient handoffs.
- Maintain accurate and detailed electronic health records (EHR) for all patient interactions.
- Provide compassionate support to patients and families regarding their health inquiries.
- Identify potential health risks and escalations, ensuring timely intervention.
- Stay updated on current medical guidelines and triage best practices.
Qualifications
- Active Registered Nurse (RN) license required (or eligibility to obtain one through our partner program).
- Strong interpersonal and verbal communication skills.
- Ability to remain calm and composed under pressure in high-stress situations.
- Basic Life Support (BLS) certification is preferred.
- Previous experience in a clinical setting or customer service role is a plus.
- Reliable internet connection and a dedicated workspace for remote or hybrid work.
