Responsibilities
- Perform comprehensive phone and online triage assessments for patients presenting with acute and chronic symptoms.
- Utilize evidence-based protocols to determine the urgency of care and facilitate timely appointments or emergency interventions.
- Provide patient education regarding health maintenance, medication management, and preventative care strategies.
- Collaborate effectively with physicians, nurse practitioners, and other healthcare providers to coordinate seamless patient care pathways.
- Document all triage encounters accurately and in compliance with HIPAA regulations and hospital policy.
- Manage high-volume call queues efficiently while maintaining a calm and professional demeanor under pressure.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of California.
- Minimum of 2 years of recent clinical nursing experience, with specific triage experience preferred.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong computer proficiency with EMR/EMR systems (e.g., Epic, Cerner, Athenahealth).
- Excellent verbal and written communication skills with the ability to convey complex medical information clearly to patients.
- Ability to work flexible shifts, including evenings, weekends, and holidays as required.
