As a Triage Nurse, you will be the first line of defense in our patient care continuum. You will utilize your clinical expertise to assess patient needs, provide medical advice, and determine the appropriate level of care. Responsibilities
- Conduct comprehensive patient assessments via phone or digital channels to determine the urgency of medical needs and initiate appropriate care plans.
- Make clinical decisions regarding patient triage levels and facilitate timely referrals to physicians, urgent care, or emergency services when necessary.
- Document all patient interactions, assessments, and triage decisions accurately and promptly in the electronic health record (EHR).
- Collaborate with healthcare providers and case managers to ensure continuity of care and patient satisfaction.
- Provide clear, empathetic, and concise health education to patients and their families regarding symptoms, medications, and home care instructions.
- Manage high call volumes while maintaining a calm demeanor and adhering to strict quality and compliance standards.
- Identify and escalate critical or complex patient issues to on-call providers immediately.
Qualifications
- Active and unencumbered Registered Nurse (RN) License in the State of California.
- Minimum of 2-3 years of experience in a clinical setting, with specific experience in triage, urgent care, or telehealth preferred.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong critical thinking skills and the ability to make sound medical judgments under pressure.
- Excellent verbal and written communication skills with the ability to de-escalate patient emotions effectively.
- Proficient in using electronic medical records (EMR/EHR) systems.
- Flexibility to work rotating shifts, including evenings, weekends, and holidays.
