Responsibilities
- Conduct comprehensive telephone and video triage assessments to determine the acuity of patient symptoms and recommend appropriate levels of care.
- Document all clinical findings, patient interactions, and triage decisions accurately and promptly within the Electronic Medical Record (EMR) system.
- Coordinate patient referrals to urgent care centers, emergency departments, or specialist appointments based on clinical necessity.
- Provide clear, empathetic health education and self-care instructions to patients and their families to promote recovery and prevent readmission.
- Collaborate closely with physicians, nurse practitioners, and care management teams to ensure continuity of care for high-risk patients.
- Follow up on complex or unresolved cases to ensure patient safety and satisfaction.
Qualifications
- Current, active Registered Nurse (RN) license in the state of Florida.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 2 years of nursing experience, with a preference for experience in triage, call center nursing, or emergency medicine.
- Strong computer literacy and proficiency with Electronic Health Records (EHR) software.
- Excellent verbal and written communication skills with a focus on patient-centered care.
- Ability to remain calm and make sound clinical judgments under pressure.
