Responsibilities
- Conduct telephone-based patient assessments using SBAR methodology
- Prioritize patient calls based on acuity and urgencyCollaborate with physicians and clinical staff for care coordination
- Document patient encounters accurately in EMR systems
- Provide education on home care and self-management
- Maintain compliance with HIPAA and regulatory standards
- Participate in quality improvement initiatives
Qualifications
- Active RN license in Kentucky (or compact state)
- Minimum 1 year clinical nursing experience (ER preferred)
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Proficiency with electronic health records
- Ability to work in high-pressure environments
- Basic ECG interpretation knowledge
- BLS certification required (ACLS preferred)
