Responsibilities
- Conduct virtual patient assessments and triage via telehealth platform
- Develop and implement personalized care plans for assigned patients
- Coordinate with multidisciplinary teams for continuity of care
- Document all patient interactions accurately in EHR systems
- Provide health education and medication management guidance
- Respond to urgent patient inquiries during weekend shifts
- Maintain HIPAA compliance in all digital communications
Qualifications
- Active RN license in Ohio (compact license acceptable)
- Minimum 2 years clinical nursing experience
- Telehealth or remote care certification preferred
- Proficiency with EHR systems (Epic, Cerner, etc.)
- Strong critical thinking and decision-making skills
- Excellent written and verbal communication abilities
- Ability to work independently with minimal supervision
- Weekend availability (Sat/Sun, 12-hour shifts)
