Responsibilities
- Provide personalized care to elderly and disabled clients in their homes
- Assist with ADLs (bathing, dressing, grooming, toileting)
- Monitor vital signs and document client health status
- Administer prescribed medications under supervision
- Coordinate care plans with registered nurses and families
- Transport clients to medical appointments as needed

Qualifications
- Active California CNA certification required
- Minimum 1 year home health or hospice experience
