Responsibilities
- Provide direct, hands-on patient care, including bathing, dressing, and feeding assistance.
- Monitor and record vital signs (blood pressure, pulse, temperature) accurately and timely.
- Assist licensed nurses with medical procedures, wound care, and medication administration under supervision.
- Communicate effectively with patients, families, and the multidisciplinary healthcare team regarding patient status and concerns.
- Maintain a clean, safe, and therapeutic environment in accordance with facility protocols and infection control standards.
- Document all patient care activities and changes in condition in electronic health records (EHR) accurately.
Qualifications
- Current and valid Illinois CNA License (Required).
- CPR and Basic Life Support (BLS) Certification (American Heart Association).
- Previous experience in a long-term care, rehabilitation, or hospital setting preferred.
- Strong interpersonal and communication skills with a focus on patient-centered care.
- Ability to stand for extended periods and lift patients (up to 50 lbs) as needed.
- Reliable attendance and the ability to work weekends and holidays as scheduled.
