Responsibilities
- Deliver high-quality virtual patient assessments and care coordination
- Monitor patient health remotely using telehealth platforms and EHR systems
- Develop and implement individualized care plans via telehealth channels
- Collaborate with interdisciplinary teams for complex patient cases
- Provide patient education and health coaching through digital platforms
- Ensure strict compliance with HIPAA and telehealth regulations
- Document patient encounters accurately in electronic health records
Qualifications
- Active RN license in the state of Florida (compact license accepted)
- BSN degree (ADN candidates with 3+ years experience considered)
- Minimum 2 years of clinical nursing experience (telehealth preferred)
- Proficiency with telehealth platforms (e.g., Teladoc, Amwell)
- Strong critical thinking and decision-making abilities
- Excellent communication skills for virtual patient interactions
- Ability to work independently with minimal supervision
