Responsibilities
- Deliver high-quality patient assessments and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Monitor patient vitals and medication adherence remotely using EHR systems
- Provide education and support to patients and families regarding chronic conditions
- Document clinical encounters accurately and maintain compliance with healthcare regulations

Qualifications
- Active RN license in Texas (or compact state)
- Minimum 2 years of acute care or telehealth nursing experience
- BLS and ACLS certifications required
- Proficiency with electronic health records (EHR) systems
- Exceptional communication skills and ability to build rapport virtually
- Strong critical thinking and decision-making abilities
- Ability to work independently and manage multiple cases efficiently
