Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platform
- Monitor and interpret patient data using advanced telehealth monitoring systems
- Collaborate with multidisciplinary teams to develop personalized care plans
- Provide patient education on chronic disease management and medication adherence
- Document accurate clinical notes in EMR systems within 24-hour turnaround
- Respond to patient inquiries via secure messaging and virtual consultations
- Participate in continuous quality improvement initiatives
Qualifications
- Active AZ RN license (compact license accepted)
- Minimum 2 years of clinical nursing experience
- Telehealth or remote care experience preferred
- BLS/ACLS certification required
- Proficiency with EMR systems (Epic preferred)
- Exceptional communication and documentation skills
- Ability to work independently in a remote environment
