Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting in a respectful and dignified manner.
- Prepare nutritious meals and assist with feeding clients who require assistance.
- Provide companionship and engage in meaningful conversations to combat isolation.
- Monitor client health and report any changes, symptoms, or concerns to the Case Manager immediately.
- Perform light housekeeping duties, including laundry and cleaning, to ensure a safe living environment.
- Accompany clients to medical appointments and assist with errands as directed.
- Administer medication reminders and document all care activities accurately in the electronic medical record.
Qualifications
- High School Diploma or GED is required.
- Current CNA, PCA, or HHA certification is preferred but not mandatory for qualified candidates.
- Valid Driver's License and proof of insurance are required.
- Ability to pass a comprehensive background check and drug screening.
- Excellent interpersonal and communication skills with a patient and empathetic demeanor.
- Physical ability to lift, stand, walk, and bend for extended periods.
- Reliable transportation and a valid Social Security Number.
