Responsibilities
- Provide direct, individualized care to patients in accordance with the patient's care plan.
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and record vital signs such as blood pressure, pulse, and temperature.
- Turn, reposition, and transfer patients to prevent bedsores and ensure patient comfort.
- Assist with meal preparation and feeding as needed.
- Observe and report any changes in a patient's physical, mental, or emotional condition to the RN or Case Manager.
- Maintain a clean, safe, and organized environment for patients.
Qualifications
- Current and valid state CNA Certification.
- Current Basic Life Support (BLS) certification required.
- Strong communication skills and the ability to interact effectively with patients and families.
- Compassionate, patient, and detail-oriented nature.
- Ability to stand for long periods and lift up to 25 lbs.
- Must pass a background check and drug screen.
- Previous experience in a home health or long-term care setting is preferred.
