Responsibilities
- Provide compassionate personal care assistance including bathing, dressing, and mobility support
- Monitor vital signs and report changes to RN case managers
- Assist with medication administration under supervision
- Document care plans accurately using our HIPAA-compliant EMR system
- Coordinate meal preparation and feeding assistance
- Facilitate therapeutic activities and social engagement
- Maintain sterile environments and infection control protocols
Qualifications
- Active NYS CNA certification in good standing
- Minimum 1 year experience in home health or long-term care
- BLS/CPR certification (or willing to obtain)
- Exceptional communication and documentation skills
- Ability to lift 50+ pounds with proper technique
- Flexibility to work varying shifts (early morning, evening, weekends)
- Passionate about geriatric care and patient dignity
- Reliable transportation for multi-site assignments
