Responsibilities
- Conduct comprehensive virtual patient assessments and monitor vital signs remotely.
- Document patient health data accurately into Electronic Health Records (EHR) systems.
- Communicate effectively with physicians, nurses, and care coordinators regarding patient status.
- Provide education and support to patients and families on managing chronic conditions.
- Respond promptly to urgent patient inquiries via secure messaging channels.
- Collaborate with local Ohio healthcare facilities to ensure continuity of care.
Qualifications
- Certified Nursing Assistant (CNA) license required in the State of Ohio.
- Current Basic Life Support (BLS) certification.
- Previous experience in home health or telehealth nursing preferred.
- Strong computer literacy and proficiency with EMR/EHR software.
- Excellent verbal and written communication skills.
- Ability to maintain strict patient confidentiality and data security.
