Responsibilities
- Provide personalized care including bathing, dressing, and feeding assistance
- Monitor vital signs and document patient progress accurately
- Assist with mobility transfers and ambulation support
- Administer medications under RN supervision
- Coordinate telehealth visits and remote care monitoring
- Maintain detailed electronic health records
- Collaborate with interdisciplinary care teams
Qualifications
- Active Texas CNA certification required
- Minimum 1 year home care or hospital experience
- CPR/First Aid certification (or willing to obtain)
- Reliable transportation with valid driver's license
- Proficient in telehealth platforms and EHR systems
- Exceptional communication and empathy skills
- Ability to work independently in client homes
