Responsibilities
- Patient Monitoring: Observe and report changes in patient conditions, including vital signs, skin integrity, and behavior to the Registered Nurse (RN) or Charge Nurse.
- Activities of Daily Living (ADLs): Assist patients with bathing, dressing, grooming, toileting, and eating as needed to ensure dignity and hygiene.
- Assistance & Mobility: Help patients move around the facility safely using walkers, wheelchairs, or transfer devices, and provide range-of-motion exercises.
- Communication: Effectively communicate with patients, families, and the healthcare team to ensure everyone is informed about patient status and needs.
- Documentation: Accurately document care provided, intake and output, and any unusual incidents in the electronic medical record (EMR) system.
- Hygiene & Environment: Maintain a clean, safe, and organized patient room environment, including linens and equipment.
Qualifications
- Licensure: Valid and current Louisiana State CNA Certification (Licensure required).
- Certifications: Current Basic Life Support (BLS) and CPR certification.
- Education: High School Diploma or GED equivalent required.
- Experience: Previous experience as a CNA in a hospital, long-term care, or home health setting preferred but not required for top candidates.
- Skills: Strong understanding of infection control protocols and patient safety measures.
- Soft Skills: Excellent interpersonal skills, patience, empathy, and the ability to work well in a team.
