Role Overview

As a CNA at CareBridge Health, you will play a vital role in our patients' daily lives. We are looking for individuals who are patient, observant, and eager to provide the highest standard of care in a modern medical setting.

Responsibilities

  • Provide direct personal care to patients, including assistance with bathing, dressing, grooming, and toileting.
  • Monitor and record patient vital signs (blood pressure, temperature, pulse, respirations) accurately and according to protocol.
  • Assist patients with mobility and transfers using proper body mechanics and mechanical lifting equipment.
  • Maintain a clean, safe, and therapeutic environment by adhering to infection control and safety standards.
  • Communicate changes in a patient's physical or mental condition to the Registered Nurse or Charge Nurse immediately.
  • Document patient care plans, daily progress, and any incidents in the Electronic Health Record (EHR) system.
  • Collaborate effectively with the interdisciplinary care team to ensure holistic patient support.

Qualifications

  • Current, unencumbered Oregon CNA License required.
  • Current Basic Life Support (BLS) certification for Healthcare Providers (American Heart Association).
  • High school diploma or General Equivalency Diploma (GED) required.
  • Strong communication and interpersonal skills with a focus on patient-centered care.
