Responsibilities
- Conduct remote patient monitoring using approved telehealth software and wearable devices.
- Assess patient vitals, symptoms, and general health status via secure video and digital communication channels.
- Communicate effectively and compassionately with patients, families, and the clinical care team.
- Document patient interactions, care activities, and changes in condition accurately in the Electronic Health Record (EHR).
- Administer medications and treatments as directed by the supervising physician or RN, following strict protocols.
- Respond promptly to patient inquiries and alerts regarding medical emergencies.
- Participate in continuous quality improvement initiatives and staff training sessions.
Qualifications
- Current, active, and unencumbered CNA License in the State of Ohio.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Proficiency in using computer systems, tablets, and telehealth applications.
- Strong interpersonal and communication skills with the ability to convey complex medical information clearly.
- High school diploma or GED required; Associate's degree in Nursing preferred.
- Ability to work independently in a remote setting with minimal supervision.
- Reliable high-speed internet connection and a private workspace.
