Responsibilities
- Conduct remote patient health monitoring and vital sign assessments via secure telehealth platforms.
- Communicate effectively with patients, families, and the wider healthcare team to ensure continuity of care.
- Maintain accurate and up-to-date electronic health records (EHR) in strict compliance with HIPAA regulations.
- Provide compassionate emotional support and health education to patients managing chronic conditions.
- Coordinate follow-up appointments, medication adherence reminders, and care plans for assigned caseloads.
- Collaborate with licensed nurses and physicians to address patient concerns and adjust care plans remotely.
- Participate in ongoing training to stay current with telehealth best practices and medical protocols.
Qualifications
- Active and unencumbered Certified Nursing Assistant (CNA) license in the State of Ohio.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred.
- Proficient in using computer systems, tablets, and telehealth software applications.
- Strong interpersonal and communication skills with a high degree of patient empathy.
- Ability to work independently in a remote environment while adhering to strict clinical guidelines.
- High school diploma or GED required; Associate degree in Nursing (ADN) or higher is a plus.
