Responsibilities
- Conduct remote patient assessments and monitor vital signs via secure telehealth platforms.
- Document patient care plans and update electronic health records (EHR) accurately and timely.
- Communicate effectively with physicians, RNs, and nursing staff regarding patient status and needs.
- Provide education and support to patients regarding their health conditions and medications.
- Ensure strict compliance with HIPAA regulations, hospital protocols, and safety standards.
- Identify potential health risks and escalate concerns to the care team immediately.
Qualifications
- Active and unencumbered North Carolina CNA License required.
- Minimum of 1 year of nursing experience preferred.
- Proficient in computer skills and electronic health record systems (Epic, Cerner).
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively in a remote setting.
- Reliable high-speed internet and a private workspace are mandatory.
