Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming assistance to residents in a home-like setting.
- Monitor and record vital signs (blood pressure, temperature, pulse) and report any abnormalities to the Nurse Manager.
- Assist with daily living activities, including meal preparation, mobility support, and toileting assistance.
- Administer medications as directed by licensed nurses and document all care accurately in electronic health records.
- Communicate effectively with residents, families, and the interdisciplinary care team to ensure holistic care.
- Maintain a clean, safe, and organized environment for residents and staff.
- Participate in quality assurance activities and care plan updates.
Qualifications
- Certified Nursing Assistant (CNA) license in the State of Wisconsin required.
- Current Basic Life Support (BLS) and CPR certification.
- Previous experience in long-term care, geriatrics, or home health preferred.
- Strong communication and interpersonal skills.
- Ability to stand for long periods and lift up to 50 lbs safely.
- Compassionate, patient, and detail-oriented demeanor.
- Reliable transportation and ability to pass a background check and drug screen.
