Responsibilities

  • Provide direct patient care and assistance with activities of daily living (bathing, dressing, grooming, and toileting).
  • Monitor and record vital signs, including blood pressure, pulse, and temperature, reporting any abnormalities immediately.
  • Assist with meal preparation and feeding for patients with special dietary requirements or dysphagia.
  • Communicate changes in patient condition, behavior, or response to treatment to the Registered Nurse or Charge Nurse promptly.
  • Maintain a clean, safe, and sanitized patient environment, adhering to infection control protocols.
  • Assist with patient mobility, transfers, and positioning using proper body mechanics and assistive devices.
  • Document patient care services accurately and timely in the electronic health record (EHR) system.

Qualifications

  • Current and active Certified Nursing Assistant (CNA) license in the State of Arizona.
  • Current Basic Life Support (BLS) and CPR certification (American Heart Association or Red Cross).
  • Previous experience in a long-term care facility or home health setting is highly preferred.
  • Strong interpersonal and verbal communication skills with patients, families, and staff.
