Responsibilities
- Deliver compassionate in-person patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient health data via our secure remote portal
- Coordinate care plans with telehealth teams during remote shift hours
- Assist with medication administration under RN supervision
- Provide emotional support and companionship to patients and families
- Participate in virtual team huddles and care coordination meetings
- Maintain accurate electronic health records during remote shifts
Qualifications
- Active Nebraska CNA certification in good standing
- Minimum 1 year recent CNA experience in home care or facility settings
- Basic computer skills for remote documentation tasks
- Valid CPR/BLS certification
- Reliable transportation for in-patient visits
- Ability to work independently during remote shifts
- Excellent communication and time management skills
- Flexibility to adapt to hybrid work model requirements
