Responsibilities
- Provide direct patient care by assisting with activities of daily living, such as bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, reporting any abnormalities to the nursing staff immediately.
- Assist patients with mobility, transfers, and positioning to ensure safety and comfort throughout the shift.
- Maintain accurate and timely documentation of patient care activities, observations, and progress notes in electronic health records.
- Communicate effectively with nursing staff, physicians, and patients/families to ensure continuity of care and address patient concerns.
- Respond promptly to patient call lights and needs, maintaining a safe, clean, and therapeutic environment.
Qualifications
- Current and valid State of Illinois CNA Certification (or willingness to obtain within specified timeframe).
- High School Diploma or GED equivalent.
- CPR and BLS Certification (American Heart Association or Red Cross) required.
- Strong interpersonal and communication skills with a focus on empathy and patient-centered care.
- Ability to stand, walk, and lift up to 50 lbs for extended periods.
- Previous experience in a hospital, LTC, or home health setting is preferred but not required for entry-level candidates.
