Responsibilities
- Provide Personal Care: Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Vital Signs Monitoring: Accurately measure and record patient vital signs such as blood pressure, pulse, temperature, and respiration rates.
- Medication Assistance: Support patients with medication reminders and administration as directed by licensed nursing staff.
- Communication: Maintain clear and concise communication with patients, families, and the healthcare team to ensure continuity of care.
- Documentation: Complete thorough clinical documentation and shift reports promptly and accurately.
- Emergency Support: Respond effectively to patient needs and assist in emergency situations when required.
Qualifications
- Licensure: Current and active Illinois CNA License (or LPN depending on role scope).
- Certification: Current BLS (Basic Life Support) certification from the American Heart Association.
- Experience: Minimum of 1 year of recent experience in a hospital, nursing home, or home health setting preferred.
- Physical Stamina: Ability to stand, walk, and lift patients for extended periods.
- Interpersonal Skills: Strong patient care skills with a positive and professional demeanor.
