Responsibilities
- Provide exceptional virtual patient support, monitoring, and care coordination.
- Assist in the development and implementation of remote patient care plans.
- Maintain accurate and up-to-date electronic health records (EHR) and documentation.
- Communicate effectively and compassionately with patients, families, and multidisciplinary healthcare teams.
- Ensure strict adherence to all state health regulations, HIPAA compliance, and privacy standards.
- Identify patient needs and escalate concerns to the appropriate medical staff promptly.
- Utilize telehealth platforms to conduct virtual assessments and follow-ups.
Qualifications
- Active and unencumbered CNA license in the State of New York.
- Minimum of 1-2 years of experience in a clinical or home health setting.
- Strong computer literacy and proficiency with electronic health records (EHR) systems.
- Excellent verbal and written communication skills.
- Ability to work independently in a remote environment with high reliability.
