Responsibilities
- Provide Personal Care: Assist clients with daily living activities, including bathing, dressing, and grooming, ensuring dignity and comfort.
- Monitor Health: Track and report vital signs, such as blood pressure and temperature, to the supervising nurse.
- Medication Management: Remind clients to take prescribed medications on time and assist with medication administration as trained.
- Companionship: Engage in conversation and activities to reduce isolation and improve the client's emotional well-being.
- Household Support: Perform light housekeeping tasks related to patient care, such as laundry and meal preparation.
- Communication: Maintain clear and consistent communication with clients and the care coordination team via phone or video calls.
- Emergency Response: Respond promptly to client emergencies and follow established safety protocols.
Qualifications
- High School Diploma: or equivalent required.
- CNA Certification: Active Certified Nursing Assistant license preferred, but not mandatory for entry-level candidates willing to train.
