Responsibilities
- Provide direct patient care, assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, reporting any abnormalities to the charge nurse.
- Assist patients with mobility, transfers, and walking using proper body mechanics and assistive devices to ensure safety.
- Collaborate with the nursing team to develop, implement, and evaluate individualized patient care plans.
- Document patient progress, care provided, and changes in condition accurately in electronic medical records (EMR).
- Maintain a clean, safe, and therapeutic environment by adhering to infection control protocols and safety standards.
- Respond promptly to patient requests and emergency situations, demonstrating a calm and professional demeanor.
Qualifications
- Certified Nursing Assistant (CNA) license required; Licensed Practical Nurse (LPN) license preferred.
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA) required.
- High school diploma or GED required; Nursing diploma or Associate Degree in Nursing (ADN) preferred.
- Previous experience in a clinical, long-term care, or hospital setting is highly preferred.
- Strong communication skills and the ability to work effectively as part of a multidisciplinary healthcare team.
- Compassionate demeanor with a strong focus on patient-centered care and empathy.
- Ability to stand for extended periods, lift up to 50 lbs, and perform physical tasks as required by the job.
