Responsibilities
- Provide direct patient care, including assistance with daily living activities and personal hygiene.
- Monitor and record patient vital signs and health status accurately and timely.
- Assist with mobility and transfers to ensure patient safety and comfort.
- Prepare and serve meals according to specific dietary restrictions and preferences.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
- Maintain a clean, safe, and sanitary patient environment at all times.
Qualifications
- Certification as a CNA or LPN (current and unencumbered license to practice).
- Previous experience in a home health or long-term care setting is preferred but not always required.
- Strong interpersonal and communication skills with a focus on patient advocacy.
- Ability to pass a criminal background check, drug screen, and health assessment.
- Valid driver’s license and reliable transportation (if required for the role).
