Responsibilities
- Provide direct patient care and assistance with daily living activities (ADLs) such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, and pulse, accurately and timely.
- Administer prescribed medications and treatments according to physician orders and established protocols.
- Communicate effectively with patients, families, and the interdisciplinary care team to ensure continuity of care.
- Document patient care plans, progress notes, and any changes in condition in the electronic health record (EHR).
- Maintain a clean, safe, and therapeutic environment for patients.
- Assist with light housekeeping and meal preparation as required to ensure patient well-being.
Qualifications
- Certified Nursing Assistant (CNA) license in the state of Georgia required.
- BLS (Basic Life Support) certification is mandatory.
- Strong interpersonal and communication skills with a focus on patient empathy.
- Ability to work flexible shifts, including nights, weekends, and holidays.
- Previous experience in home health or hospital nursing preferred.
- Ability to stand for long periods and lift up to 50 lbs as needed.
- Reliable transportation and a valid driver's license.
