- Provide remote telehealth monitoring and virtual patient assessments
- Administer medication reminders and track health vitals via digital platforms

- Respond to patient inquiries through secure messaging systems
- Participate in virtual team meetings and continuing education
Qualifications
- Active New Mexico CNA certification
- Minimum 1 year clinical experience (telehealth preferred)
- Reliable high-speed internet and quiet home workspace
- Proficiency with EHR systems and telehealth software
