Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs), including bathing, dressing, and grooming.
- Monitor vital signs (blood pressure, temperature, pulse) and report any significant changes to the RN/LPN supervisor immediately.
- Assist with mobility, transfers, and positioning to ensure patient safety and comfort.
- Maintain a clean, safe, and therapeutic environment in accordance with facility standards.
- Document patient care plans, daily updates, and any incidents accurately in electronic medical records.
- Communicate effectively with patients, families, and the interdisciplinary care team.
Qualifications
- Current and valid California CNA License or Geriatric Nursing Assistant (GNA) certificate.
- Basic Life Support (BLS) certification required (ACLS preferred but not mandatory).
- High school diploma or GED equivalent.
- Strong interpersonal and communication skills.
- Physical stamina to stand, walk, and lift for extended periods.
- Previous experience in a long-term care or home health setting is a plus.
