Responsibilities
- Deliver high-quality patient care including bathing, dressing, and feeding assistance
- Monitor vital signs and document patient observations accurately
- Assist with mobility transfers and use of medical equipment
- Coordinate with healthcare team via telehealth platforms for remote case management
- Administer medications under RN supervision per facility protocols
- Maintain clean, safe patient environments and infection control standards
- Provide emotional support to patients and families during care transitions
Qualifications
- Active Virginia CNA certification in good standing
- Minimum 1 year of recent CNA experience in long-term care or hospital setting
- Valid CPR certification and TB test clearance
- Proficient in EHR documentation and telehealth software
- Ability to lift 50+ pounds and stand for extended periods
- Strong communication skills with remote team capabilities
