Responsibilities
- Perform Comprehensive Assessments: Conduct detailed phone and video evaluations to accurately determine patient acuity and appropriate care pathways.
- Triage Urgent Needs: Differentiate between emergent, urgent, and non-urgent conditions to guide patients to the correct level of care.
- Facilitate Referrals: Coordinate seamless transitions to primary care providers, specialists, or urgent care facilities as needed.
- Patient Education: Provide clear, evidence-based medical advice and health education to alleviate patient anxiety and ensure understanding.
- Documentation: Record all triage interactions and clinical decisions in the Electronic Health Record (EHR) with precision and timeliness.
- Quality Assurance: Participate in regular audits and feedback sessions to maintain high clinical standards.
Qualifications
- Licensure: Current, unencumbered Registered Nurse (RN) license to practice in the State of Nebraska.
- Certifications: Current BLS and ACLS certifications required.
- Experience: Minimum of 2-3 years of nursing experience, preferably in a triage, call center, or urgent care setting.
- Skills: Strong critical thinking skills with the ability to make sound clinical decisions under pressure.
- Communication: Excellent verbal and written communication skills with a patient-centered approach.
- Technology: Proficiency with EMR systems and telehealth platforms.
