Responsibilities
- Conduct thorough patient assessments via phone, email, and patient portal to determine the severity of medical needs.
- Apply clinical judgment to prioritize care, providing appropriate medical advice and triage instructions.
- Evaluate symptoms, medical history, and vital signs to make informed clinical decisions.
- Educate patients and caregivers on self-care management and medication instructions.
- Coordinate patient care by facilitating appointments with providers or specialists as necessary.
- Document all patient interactions and triage decisions accurately and timely in the electronic health record (EHR).
- Escalate critical cases immediately to on-call providers or emergency services.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of California.
- Minimum of 2 years of recent acute care nursing or triage experience.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong computer proficiency with experience in EMR/EHR systems.
- Excellent verbal and written communication skills for clear patient instruction.
- Ability to remain calm under pressure and make quick, accurate clinical decisions.
- Commitment to patient privacy and HIPAA compliance.
